The train was going too fast; the Westinghouse brake failed. When they tried to apply the handbrakes, it was too late. One woman on the street was killed; she was selling papers, standing in for her husband (the train company paid a pension for their children’s welfare). There were five other serious injuries.
The driver and conductor were prosecuted. Seems that at least one of them was too busy doing paperwork to act quickly enough and avoid the disaster.
